看起来你在DOCTYPE等之前的顶部错过了'?>'。
表单有no action=""所以把当前php文件的名称或$ _SERVER ['PHP_SELF']
学生姓名选项和警报输入没有结束标记
尝试在提交按钮上执行isset，不需要隐藏输入：

if(isset($_REQUEST['submit'])) {或if(isset($_POST['submit'])) {

如果它仍然无效，请在输出表单之前将整个代码块移到顶部。

不要使用empty（）来检查它是否已设置：

if(isset($_POST['alert']))
    $alert = $_POST['alert'];
else 
    $alert = "";

或简称：$alert = isset($_POST['alert'])?$_POST['alert']:"";

而不是：
