似乎无法使用php将数据插入到我的数据库中

我现在是Web开发的新手，所以我看不出我的代码有什么问题。问题是我无法通过PHP将数据从我的网站插入数据库。我的IDE和浏览器没有弹出错误或警告消息，因此我无法指出为什么我不能添加数据。我还在样式表中使用Bootstrap。
