您的PHP和HTML表面看起来不错。似乎您缺少内容，因为生成PDF的按钮未传递$id变量。您至少应该将表单修改为如下所示：

此外，您实际上并没有对表单做任何事情（即不传递表单字段），因此也可以将其链接。使用Bootstrap这样的UI框架时，操作起来稍微容易一些，因为您可以设置链接的样式，使它们看起来像按钮。

对于CSS，这实际上取决于您如何引用外部文件（如果是外部文件）。这将有助于查看要提供给Dompdf的HTML，以调试问题的这一部分。
