For Canadian residents. record the 6-character postal code. Do not record hyphens. See FORDS, for additional information.|
5|5|Record the county of the patient's usual residence when the tumor was diagnosed. This data item is required for residents of the reporting insitiution's state ONLY. Enter the correct three digit county code. Enter 998 is the patient resides outside of the state of your institution, 999 if the county is unknown. 7|7|Identifies the date of birth of the patient. Enter the patient's date of birth. Enter each of the date fields (MMDDYY); Use 99 to identify unknown month or day, and 99999999 representing an entirely unknown date. If year is known but unknown month and day, enter 99/99/1995. (Remember to use "]" if part of a name is used). Select the correct physician by highlighting the appropriate code then either double clicking on the highlighted line or selecting "OK" with the mouse.
