您的代码存在一些问题：

您将错误的参数传递给mail()。这一行：

mail($to, $subject, $name, $email, $headers);

应该是：

mail($to, $subject, $message, $headers);

您没有存储所有表单字段：

您需要从$_POST检索所有表单字段并将其合并到您的邮件中。也许是这些方面的事情：
